Restraint Log App

Free resource

Record physical interventions clearly, safely and professionally — built to support Ofsted & CQC-ready incident reporting.

This tool supports recording — it does not replace your incident report, safeguarding procedures, Regulation 40 / CQC notifications, local authority reporting, medical advice or professional judgement. Physical intervention should only ever be used when necessary, proportionate, reasonable and the least restrictive option in response to immediate risk. Only use techniques you have been trained and assessed as competent to use.

Demo scenario

A young person becomes distressed following a boundary around leaving the home late at night. Staff use verbal reassurance, offer space, reduce the audience, offer choices and attempt distraction. The young person attempts to strike another young person and moves towards the front door. Staff complete a dynamic risk assessment and use a trained low-level touch support and escort to move the young person to a safer area. The intervention lasts 45 seconds and is released as soon as the risk reduces. The young person is offered emotional support afterwards, and the manager reviews the incident.
